ADOBE PRINTING INSTRUCTIONS
You will need to print page (2) on the back of page (1). Make sure you feed page one back through the printer so the sections containing the information that you need to send us is lined-up on the front & back of the form.
After Adobe Reader Opens...
Click the print icon "on the adobe toolbar (top left).
Under the "Print Range" options, toggle (select) the "Pages" option & backspace 1-2 then enter "1" to print just page one
After page one is printed - Backspace 1-2 then enter "2" to print just page two. (Page two will be displayed in the preview pane).
Note: Make sure the "Print to File" option box is not checked. ◄
