How to register using a print &
mail form:
A registration form can be printed from this
website by clicking on the link at the bottom of this page. The form will then
need to be sent by mail to the Lancaster address displayed on the
registration form, (and at the bottom of this page). A separate form must be
filled out for each player. Payment by check or money order can be remitted with
the registration form(s) made payable to LYSC. If you are interested in
coaching, you can fill out a form, or contact us by
email.
Printing the form:
The participant registration
form link at the bottom of this page will open the registration form using your
Adobe Reader Program. Allow 10-30
seconds for your program to open and the form to be fully displayed. If Adobe
Reader is not installed on your PC, you can download the latest version for
free. The download time can be in excess of
one hour if you have a dial-up connection. A DSL or Cable connection only takes
a few minutes.

You will need to print
page (2) on the back of page (1). Make sure you feed page one back through
the printer so the sections containing the information that you need to send us is lined-up
on the front & back of the form.

Instructional (U-6)
Parents/Guardians:

There is not a separate option to
select U-6 on the front of the form. Please use the
Coed
Instructional (U-8) $30 before June 1 or Dec.1, and $35 after, option. The cost for
the instructional programs are the same and there is an area on the back of
the form where you can enter (U-6) as the age division.
Please Note:
The deadline to register to receive
the $5.00 discount is June. 1st for fall registration., and Dec 1st for
spring registration.
